This article addresses some developments of the psychoanalytic concept of self in Kohut's theory, with the aim of understanding the challenges encountered by the contemporary psychoanalytic clinic in the treatment of patients with narcissistic personality disorders. To this end, excerpts from three clinical cases will be used, articulating them with the concept of self. We consider that the treatment of subjects with narcissistic personality disorders highlights the limits of clinical practice. In this sense, the three clinical cases will be analyzed with an emphasis not on the successes, but, above all, on the failures in the clinical management. Thinking about these failures allowed us to reflect on what would have contributed to the stagnation or interruption of the treatment of these patients, for example: the analyst's lack of resources, the possibilities and impossibilities of repairing the marks left on the self by excessive and primary environmental failures.
